How To Order
In order to take advantage of this offer,
please follow our 3-step verification process:
1. Download the form below and fill out your order information. No charges will be placed on your credit card until we have confirmed your order details with you.
2. Send your completed form, along with at least one of the eligibility documents listed below, to ordermanagement@humantouch.com. Please allow 1-2 business days processing time while we review your documentation.
3. Once we are able to verify your documentation, we will send you an order confirmation and process your payment at that time.

• Paystub less than 60 days old
• 1099 tax form from previous year
First Responder Eligibility
• Employee or volunteer badge
• Paystub less than 60 days old
• 1099 tax form from previous year
• Signed affidavit issued from first responder agency
Active Military and Veteran Eligibility
• Veterans designated driver's license
• Veteran ID Card (VIC)
• Current Paystub (for active military)
